PLM Families Together (PLMFT) provides housing and comprehensive services for homeless families in Wake County, including assessment, short-term transitional housing with intensive support services, permanent housing placement, and after case management.

WHY WE ARE UNIQUE:
- Families are housed for 3-4 months in 28 individual apartments at 2 apartment complex locations that are integrated into residential neighborhoods. This maximizes privacy, allows PLMFT to accommodate any blend of family members, helps children and teens in those families have as “normal” a living experience as possible, and increases families’ successful and quick return to stable housing in the community.
- We have an 80% success rate – 80% of families who complete the program move into stable housing. We follow graduates for up to one year in “after care” to help prevent a return to homelessness. Data on retention rate is being collected. After care program started in February, 2008.
- We are one of very few housing options for homeless families with fathers and teenage boys. We are the only program with 3-bedroom units for large families.
- Families receive professional social work services through our team of “Mentor Advocates,” who help families set clear goals building on strengths, focus on increasing income through employment, and connecting families to resources and programs in the community that they can continue to access once they graduate from PLMFT into permanent housing.
- Families must save in an escrow account, to build financial discipline.
- Families participate in regular life skills classes hosted by local churches.
- Children receive special attention through the After School Program.
- 28 year legacy of service and leadership in family homelessness.
OUR PROGRAM:
3 – 4 months (16 week maximum limit) at no cost to the client
Housing with intensive case management services provided by professional Mentor Advocates. Priority focus on increasing income through employment and savings, and achieving stable housing.
- Individual apartment units, for maximum privacy, 1, 2, and 3 bedroom units
- Saving in an Escrow Account required (there is no other cost or program fee charged)
- Comprehensive resource and referral, in-home case management (person-centered planning; goal setting; trauma informed staff)
- Life Skills Classes held at least monthly, on topics including household budgeting, consumer credit, tenants’ rights and responsibilities, eviction and housing law, finding and keeping employment, pediatric health and family wellness, nutrition and fitness, preventive health screenings
- After School Program for tutoring and enrichment for elementary aged children
- Developmental screening provided on site for all children ages 0-5
- Other services include transportation, food and food referrals, household items, school supplies, holiday gifts
- After care services available for up to one year after graduation from the program
